RN Case Manager

Registered Nurse Case Manager

Location: Bozeman, MT

Job Type: Contract | 8x5 days.

Contract Length: 13 Weeks with possible extension

Pay Range: $2800 - $2920 Gross Weekly

Key Responsibilities:

• Complete comprehensive assessments within regulated timelines to determine eligibility for case management services.
• Collaborate with patients, caregivers, physicians, and other healthcare professionals to develop individualized care plans.
• Monitor care plans for effectiveness, document interventions, and adjust strategies as needed.
• Address and resolve barriers that hinder effective patient care and timely discharge planning.
• Promote integration of services, including behavioral health, long-term care, and community resources.
• Maintain regular outreach and management of assigned member caseloads.
• Support safe, effective, and patient-centered discharge planning.

Requirements

• Current Montana RN Licensure (required)
• Basic Life Support (BLS) Certification from the American Heart Association (required)
• Minimum of 2 years nursing experience (required)

Preferred Qualifications:

• Bachelor’s Degree in Nursing (BSN)
• 2+ years of acute care hospital experience as a Case Manager
